Complaint Investigation Summary Report #: 5105 <br />COMPLAINT ID: COMATUSD Site Location: LEPRINO FOODS 2401 N. MACARTHUR DR., TRACY <br />Activity Summary <br />Activity Date Recorded by <br />07/05/2023 On June 27, 2023, at approximately 0745 a maximum of 70 pounds, as estimated SUSZYCKI <br />by the facility, of anhydrous ammonia was released from a loose gasket. <br />According to the facility representative, David Chitwood, the release was first <br />detected by a delivery driver and had been stopped by 0809. Facility employees <br />were able to discover the location of the leak using sulfur sticks as indicators. <br />Gas/air meters did not detect any observable amount of anhydrous ammonia in <br />the air. According to David Chitwood, there were no injuries or evacuations <br />associated with the ammonia release. <br />Please submit the following documentation to the San Joaquin County <br />Environmental Health Department by August 5, 2023: <br />An incident investigation report detailing the root cause of the incident and any <br />recommendations to prevent future releases. <br />Calculations used to determine the amount of anhydrous ammonia released. <br />Photographs of the area where the leak occurred. <br />Photographs of any other pertinent parts or equipment. <br />07/07/2023 Received the pictures and documentation from Mr. Chitwood. Abated the SUSZYCKI <br />complaint. Made a note in PR0529998 <br />5105.rpt <br /> Page 2 of 2
