Date: <br />Manufacturer training: <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />On site for the following inspections: <br />-Verified a copy of the Health and Safety Plan (HASP) was on the jobsite for review. <br />-While onsite EHD was informed that the newly found steel tank is not 500 gallons and was determined to be 1,500 gallons. <br />-Verified that the tank was inerted with dry ice after the triple rinsing process took place at the 30-pounds of dry ice per <br />1,000-gallon volume rate. According to the receipt observed onsite, 45 pounds of dry ice was used to complete this process. <br />-Verified the hazardous waste manifest for the rinsate that was generated. <br />-The O2 reading was 0.7%. LEL reading was 0%. Readings were taken by AdvancedGeo, Inc and approved by the Fire <br />Department. <br />-Witnessed spray painting of the tank ID (039-0520916) on the end and side of the tank. <br />-Soil sampling was conducted by AdvancedGeo, inc. <br />-Excavation size is 18ft x 12ft x 11ft. <br />- Two samples, one at each end were taken into native soil below the tank, labeled T1W and T1E. TThe soil samples obtained <br />from the tank pit were described to be sandy soil. The soil samples beneath the tank were obtained with an excavator. <br />Samples were collected at a depth of 11 ft. Groundwater and slab were not observed during excavation. <br />-Soil stockpile : approximately 75 cubic yards <br />-Three stockpile samples were taken, labeled S1, S2, and S3. <br />- No noticeable staining or discoloration was observed. <br />-Discussed sampling and analytic requirements with the sampler, referencing the sampling protocols approved in the permit. <br />-Verified EHD’s Underground Storage Tank Disposition Tracking Record Sheet was accompanying the tank as it was being <br />hauled away. <br />Note: The hazardous waste hauler company has been changed to Atlas Environmental Solutions, Inc.
