A body a t facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not <br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, <br />containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC <br />119314— The paper towel dispenser in the restroom was not touchless. Replace the current paper towel holder with a <br />mounted, touchless paper towel dispenser. Provide EHD with photographic evidence. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317 —The "private room" did not consist of <br />baseboards. Ensure the private room has baseboards, provide photographic to EHD. <br />33. All surfaces and objects in the procedure area, including chairs, armrests, tables, countertops, and trays, shall be in good <br />repair and capable of being disinfected. All counter surfaces and service trays in the procedure area shall have a smooth, <br />durable, and nonabsorbent finish. HSC 119314 — The procedure chair in the piercing room was torn/ damaged. Repair or <br />replace the procedure chair and provide EHD with photographic evidence. <br />35. Operation <br />and employee <br />training <br />records shall be maintained for 3 <br />years and made available for inspection. A written <br />Infection Prevention and Control Plan <br />(IPCP) shall be maintained, followed and updated. HSC 119313 —A training record for <br />the IPCP was <br />not available. <br />Ensure <br />all practitioners are trained on <br />the IPCP, provide training record to the EHD. <br />36. Design plans shall be submitted to EHD for approval prior to constructing or remodeling body art facility. HSC 119312 — <br />Design plans were not submitted to the EHD for approval prior to constructing the body art facility. All future <br />constructing or remodels should be authorized/ approved by the EHD. <br />37. A body art <br />facility shall not conduct business without a valid health permit. A person shall not pertorm body art without a valid <br />registration and a permitted facility. A body art facility shall notify the LEA in writing within 30 days of resignation, termination of a <br />practitioner. Anew hire or apprentice must be registered the EHD prior to performing body art at the facility. HSC 119312 & <br />119306 — The body art facility was conducting business without a valid health permit. Jose Carballo and Amanda Brown <br />are not currently permitted by the EHD. Ensure all practitioners are registered with the EHD. <br />Correct all observations and provide evidence of correction to the EHD by <br />August 294 2022. <br />Reinspection <br />onlabout: <br />A reinspection fee <br />of $156 <br />per <br />hour <br />may <br />be <br />charged. <br />Page 3 <br />of 4 <br />EH -11/17 <br />
