Monitoring records <br /> include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance <br /> records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs <br /> of all readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6)results of <br /> inventory readings and reconciliations. <br /> CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the <br /> last three years on site. Submit copies of any missing work orders and alarm records to the EHD. <br /> This is a Class II violation. <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to check and note the correct test dates for the Secondary <br /> Containment Test on multiple of the designated operator inspection reports. The incorrect dates of 11/10/2022 and <br /> 11/11/2023 was on the reports. The correct date for Secondary Containment Test is 11/11/2022. <br /> REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and <br /> maintenance for the UST system have been completed and document the dates they were done. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> This is a repeat violation, Class Il. <br /> 318 CCR 2635, 2637.1,2665, 2712 Failed to meet spill container requirements. <br /> OBSERVATION: The 87 and diesel spill container failed when tested. <br /> REGULATION GUIDANCE:All LISTS shall be equipped with a spill container that collects any hazardous <br /> substances spilled during delivery operations,that has a minimum capacity of five gallons and a drain valve that <br /> allows drainage of the collected spill into the primary container or provide a means to keep the container empty. <br /> CORRECTIVE ACTION: The spill container gasket was replaced and retested during the inspection. <br /> This is a repeat violation, Class ll. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> FA0003284 PR0231118 SCO01 09/20/2023 <br /> EHD 23-01 Rev.12/06/2021 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
