The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use <br />paper towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) — <br />The paper towel dispenser did not contain paper towels. Ensure the paper towel dispenser is supplied with paper <br />towels. <br />18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are <br />available for inspection. HSC 119303 — The client consent form and medical questionnaire did not include all of the <br />information listed in Form B. Include the following on the forms and provide a copy to the EHD: <br />Consent Form <br />- Client is at least 18 years of age. <br />- Notice that inks are not FDA approved and health consequences are unknown. <br />Medical Questionnaire <br />- Requirements for antibiotics prior to surgery or dental procedures. <br />25. Disinfectant shall be used according to the manufacturer's instructions. HSC 119309(b) —The wet contact time given for <br />the disinfectant was 1 to 3 mins. Per manufacturer's instructions, the wet contact time for the product is 2 mins. Ensure <br />the manufacturer's instructions are followed. <br />27. Products applied to skin prior to tattooing or application of permanent cosmetics, including, but not limited to, stencils, <br />marking and transfer agents, shall be single use and discarded into a waste container at the end of the procedure. HSC <br />119311(a) — The marking pencil is not single use. Ensure single use marking pencils/ pens are used. <br />31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not <br />share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, <br />containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC <br />119314 — The restroom did not have paper towels in the paper towel dispenser and containerized liquid soap. Ensure <br />the restroom is adequately supplied with paper towels and liquid soap. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to <br />floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317 —The garbage container was not lined. Ensure the <br />garbage container is lined at all times. <br />Correct all observations and pay all applicable fees prior to scheduling the <br />final inspection. <br />Reinspection on/about: A reinspection fee of $162 per hour may be charged.
