The procedure area shall be equipped with a sink with hot and cold running water, containerized liquid soap, and single use paper towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(b) — The paper towel dispenser did not contain paper towels. Ensure the paper towel dispenser is supplied with paper towels. 18. The facilities client consent form and medical questionnaire must include all of the information listed in Form B, and are available for inspection. HSC 119303 — The client consent form and medical questionnaire did not include all of the information listed in Form B. Include the following on the forms and provide a copy to the EHD: Consent Form - Client is at least 18 years of age. - Notice that inks are not FDA approved and health consequences are unknown. Medical Questionnaire - Requirements for antibiotics prior to surgery or dental procedures. 25. Disinfectant shall be used according to the manufacturer's instructions. HSC 119309(b) —The wet contact time given for the disinfectant was 1 to 3 mins. Per manufacturer's instructions, the wet contact time for the product is 2 mins. Ensure the manufacturer's instructions are followed. 27. Products applied to skin prior to tattooing or application of permanent cosmetics, including, but not limited to, stencils, marking and transfer agents, shall be single use and discarded into a waste container at the end of the procedure. HSC 119311(a) — The marking pencil is not single use. Ensure single use marking pencils/ pens are used. 31. A body art facility shall be separated from any residential areas used for sleeping, bathing, or meal preparation and shall not share a common entrance or toilet facility with residence. The toilet facility must have a sink with hot and cold water, containerized liquid soap, and single -use paper towels that are dispensed from a wall -mounted, touchless dispenser. HSC 119314 — The restroom did not have paper towels in the paper towel dispenser and containerized liquid soap. Ensure the restroom is adequately supplied with paper towels and liquid soap. 32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, and washable. The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceiling to floor, from nail and hair activities and be separated from all business not related to body art. Approval of co -located body art facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a procedure. Each procedure area shall have lined waste containers. HSC 119314 & 119317 —The garbage container was not lined. Ensure the garbage container is lined at all times. Correct all observations and pay all applicable fees prior to scheduling the final inspection. Reinspection on/about: A reinspection fee of $162 per hour may be charged. Page 2 of 3 EH -11/17
