S A N J 0 A Q I I I Irl Environmental Health Department <br /> C o u N T AMENDED <br /> 8/28/2023 <br /> Underground Storage Tank Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> KETTLEMAN CHEVRON 601 E KETTLEMAN LN, LODI JAugust 28, 2023 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The following issues were observed with the designated operator monthly inspection reports: <br /> -inspections were not performed every 30 days(inspections were not performed between 11/2022-02/2023 <br /> -the 2/22/23 and 3/9/23 Q1, Q2, and Q3 alarms were not addressed <br /> -the 9/2022- 10/2022 reports were not signed by the owner/operator <br /> -the 03/2023-07/2023 reports did not provide the date the reports were provided to the owner and were not signed <br /> by the owner/operator <br /> REGULATION: During the inspection,the designated operator shall review the alarm history since the previous <br /> inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the alarm <br /> history with documentation taken in response to any alarms to the inspection report. Within 48 hours of the <br /> completion of the designated UST operator visual inspection,the designated operator shall sign and provide the <br /> owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the <br /> report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign <br /> and date the report, acknowledging the identified compliance issues. The designated operator visual inspections <br /> shall be conducted within 30 days or less of the previous inspection. <br /> CORRECTIVE ACTION: Ensure that all designated operator inspections are performed within the required time <br /> frames. Ensure that designated operators performing inspections at this facility are including all of the required <br /> information on the reports. During the inspection, the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. Ensure all <br /> designated operator inspection reports are completed and signed within the required time frame in the future. <br /> This is a minor violation. <br /> 302 CCR 2630(d), 2641(a)Leak detection equipment not installed/maintained/located to detect leaks at earliest <br /> opportunity. <br /> OBSERVATION: The 87 east and 91 sensors were not located to detect a leak at the earliest opportunity. <br /> REGULATION GUIDANCE: Monitoring equipment shall be maintained to be able to detect a leak at the earliest <br /> possible opportunity. <br /> CORRECTIVE ACTION: The sensor was relocated to the lowest point of the sump by the service technician. <br /> Ensure that all monitoring equipment is maintained and located to detect a leak at the earliest opportunity. <br /> This is a Class II violation. <br /> FA0003803 PR0231348 SCO01 08/28/2023 <br /> EHD 23-01 Rev.12/06/2021 Page 7 of 9 Underground Storage Tank Program OIR <br /> 1868 E.
