HSC 25284; CCR 2711(d) Failure to obtain a permit to operate from the EHD including but not limited to unpaid permit fees. • GV Express Mart INC, will insure all operating fees going forward will be paid on time and will be present onsite to hinder any disruptions. CCR 2638, 2641(j), 2715(f) Failure to have properly qualified service technician test leak detection equipment every 12 months. • GV Express Mart INC, going forward will schedule all testing prior to the due date to avoid any issues. CCR 2712(i) Failure to retain a paper or electronic copy of the permit to operate at the facility. • GV Express Mart INC, once a renewed copy of operational permit is received, it will always be posted and visible. CCR 2712(b)(1-2) Failure to maintain monitoring records for 36 months and maintenance records for 60 months. OBSERVATION: Monitoring records for the last three years were not found on site. The 2022 monitoring system certification and spill container test results were not found on site. • GV Express Mart INC, had missing records for 2022. GV going forward has hired a new company to consistently have monthly checkups and will monitor to make sure all are done on time. OBSERVATION: The following issues were observed with the designated operator monthly inspection reports: -inspections were not performed every 30 days (inspections were not performed between 11/2022 - 02/2023 -the 2/22/23 and 3/9/23 Q1, Q2, and Q3 alarms were not addressed -the 9/2022 - 10/2022 reports were not signed by the owner/operator -the 03/2023 - 07/2023 reports did not provide the date the reports were provided to the owner and were not signed by the owner/operator • GV Express will properly train manager/employees to guarantee all reports are signed, alarms are regularly monitored and reported.
