Environmental Health Department <br />Hazardous Materials Program Inspection Report <br />Date: <br />August 29, 2023 <br /> Facility Address: <br /> 7373 WEST LN, STOCKTON <br /> Facility Name: <br /> KAISER PERMANENTE <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br />OBSERVATION: The Hazardous Materials Inventory forms for diesel, liquid nitrogen, and liquid oxygen were not <br />complete. The max daily amounts and unit of measure are incorrect. 150 gallons of diesel is being reported but <br />there is 12,150 gallons of diesel on site. oxygen gas is being reported at 5,500 cubic feet but should be reported as <br />liquid oxygen at 6,100 gallons for the max daily amount. Liquid nitrogen is bing reported at 500 cubic feet but there <br />is 300 gallons of liquid nitrogen on site. List the max daily amount for liquid nitrogen at 300 gallons. Nitrous oxide is <br />being reported at 1,200 gallons but the max daily amount should be listed at 1,200 pounds. <br />REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br />of the business plan. <br />CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br />OBSERVATION: The site map was not complete. The site map is missing entrance and exits to all of the buildings. <br />Emergency response equipment ( fire extinguisher, eye wash, safety shower). The site map should be for each floor <br />of the buildings that are multiple stories. <br />REGULATION GUIDANCE: A site map shall contain a north orientation, loading areas, internal roads, adjacent <br />streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br />material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as “NA". <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br />This is a repeat violation, Class II. <br />FA0002602 PR0520139 SC001 08/29/2023 <br />EHD 19-01 Rev. 12/06/2021 Hazardous Materials Program OIRPage 2 of 3 <br />1868 E.
