<br /> The following requirements have been identified as pertinent to this project. Other requirements may
<br /> also apply. These requirements cannot be modified.
<br /> 1. Any imported fill shall be free of contaminants and meet the California Regional Water Quality
<br /> Control Board and Department of Toxic Substances Control standards prior to grading. Please
<br /> contact Howard Hold by email at Howard.Hold@waterboards.ca.gov or by phone at (916) 464-
<br /> 4679 at the State of California Water Board, Region 5, to obtain approval prior to grading.
<br /> 2. Deposition of solid waste onto land will require a solid waste notification or permit with the EHD
<br /> LEA prior to start of activities. Solid waste means all putrescible and non-putrescible solid,
<br /> semisolid, and liquid wastes, including garbage, trash, refuse, paper, rubbish, ashes, industrial
<br /> wastes, demolition and construction wastes, abandoned vehicles and parts thereof, discarded
<br /> home and industrial appliances, dewatered, treated, or chemically fixed sewage sludge which is
<br /> not hazardous waste, manure, vegetable or animal solid and semisolid wastes, and other
<br /> discarded solid and semisolid wastes (Public Resources Code 40191). Solid waste includes inert
<br /> debris such as concrete, asphalt, brick, and crushed glass (Title 14 CCR 17388). Solid waste
<br /> includes nonhazardous petroleum contaminated soil (Title 14 CCR 17361).
<br /> Any use (e.g., grading) of gravel, rock, soil, sand and similar, whether processed or not, that has
<br /> never been used in connection with any structure, road, parking lot, or similar use does not require
<br /> a solid waste notification or permit with the Environmental Health Department Solid Waste Local
<br /> Enforcement Agency (EHD LEA) (Title 14 California Code of Regulations (CCR) section
<br /> 17388.2(a)(1)).
<br /> 3. Any existing wells or septic systems to be abandoned shall be destroyed under permit and
<br /> inspection by the EHD (San Joaquin County Development Title, Section 9-605.010 & 9-601.020).
<br /> 4. Submit Water Provision Declaration form to the Environmental Health Department for review.
<br /> Note: The submitted Water Provision Declaration form is incomplete. Please include the source
<br /> for the water supply for the temporary restroom and/or office and the number of employees that
<br /> will served by an on-site well. The applicant may contact Natalia Subbotnikova, Program
<br /> Coordinator Small Public Water System Program at (209) 468-0338 with any questions regarding
<br /> the Water Provision Declaration form.
<br /> 5. No permanent structures are proposed for this project. Any installations of structures including
<br /> restrooms with plumbing fixtures, shall require the construction an approved onsite wastewater
<br /> disposal system and an approved potable water supply well for all employees (San Joaquin
