COMPLAINT INSPECTION REPORT <br /> 1:18 am <br />12:11 pm <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: PIZZA HUT #758534 Date: 07/20/2023 <br />Address: 920 N MAIN ST, MANTECA 95336 <br />Owner/Operator: PACPIZZA, LLC Telephone: (209) 368-5100 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0055998 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT STATE THAT SHE BOUGHT 4 PIZZAS (PEPPERONI, SAUSAGE, CHICKEN & PINEAPPLE & CHEESE), <br />WINGS ( HOT & CHILI HABANERO), AND 2 ORDERS OF BREAD STICKS ON 07/17/2023 AT 9 PM FROM THE <br />FACILITY. COMPLAINANT STATES THAT HERSELF AND 5 OTHER FAMILY MEMBERS INCLUDING A 4 YEAR OLD <br />CHILD, BEGAN FEELING NAUSEOUS AND HAD DIARRHEA BETWEEN 2-4 HOURS AFTER EATING. COMPLAINANT <br />STATES THAT SHE WENT TO THE DOCTORS. COMPLAINANT ALSO STATES THAT SHE IS STILL SUFFERING <br />FROM SYMPTOMS. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br /> #1 Demonstration of Knowledge <br />OBSERVATIONS: 2 valid Food handler cards are not available on site. One employee is required to have a food handler <br />card. Provide all food handler cards within 30 days. <br />CALCODE DESCRIPTION: All food employees shall have adequate knowledge of and be trained in food safety as it relates to their <br />assigned duties. (113947) Food facilities that prepare, handle or serve non-prepackaged potentially hazardous food, shall have an <br />employee who has passed an approved food safety certification examination. (113947-113947.1) Any food handler hired after June 1, <br />2011 shall obtain a Food Handler Card within 30 days (113948). <br /> #14 Food Contact Surfaces Sanitized or Warewashing Sanitization <br />OBSERVATIONS: Sanitizer buckets chlorine are at 50 ppm. Provide chlorine 100 ppm today. Corrected on site. <br />CALCODE DESCRIPTION: All food contact surfaces of utensils and equipment shall be clean and sanitized.
