Environmental Health Department <br /> Time In: 11:25 am <br /> Time Out: 11:40 am <br /> COMPLAINT INSPECTION REPORT <br /> Name of Facility: TACO BELL#35900 Date: 08/15/2023 <br /> Address: 608 W LODI AVE, LODI 95240 <br /> Owner/Operator: BRENT FLYNN Telephone: (209)369-6656 <br /> Program Element: 1600- FOOD PROGRAM Complaint#: C00056047 <br /> Responsible Party Contact: <br /> DESCRIPTION OF COMPLAINT ALLEGATIONS <br /> The San Joaquin County Environmental Health Department received the following complaint: <br /> COMPLAINANT STATES THAT ON 07/30/2023 SHE ORDERED THE BUILD YOUR OWN CRAVING BOX; GRILLED <br /> CHEESE BURRITO, SOFT TACO SUPREME, BEEFY MELT BURRITO,AND CHIPS WITH NACHO CHEESE SAUCE. <br /> COMPLAINANT STATES WITHIN 45 MINS OF EATING SHE BEGAN FEELING SEVERE CRAMPING AND DIARRHEA. <br /> COMPLAINANT STATES SHE DID NOT SEE A DOCTOR BUT DID CALL THE ADVICE NURSE AND THAT THEY <br /> SYMPTOMS LASTED 3 DAYS. <br /> VIOLATIONS AND CORRECTIVE ACTIONS <br /> Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br /> 113700.All violations must be corrected within specified timeframe. Manager states numerous other orders of the taco craving box ordered <br /> during the last couple of weeks and no similar complaints have been received.
