Facility Number: <br /> SOUD WASTE FACILITY PERMIT 39-AA-0024 <br /> a. 17. Enforcement Agency(EA)Conditions: <br /> b. This facility shall comply with all teems and conditions of this permit. <br /> c. The operator shall comply with all State Minimum Standards for solid waste handling and disposal as specified in Title 14, <br /> California Code of Regulations <br /> d. The design and operation of this facility must comply with all applicable state minimum standards for solid waste handling and <br /> disposal. <br /> c. Additional information concerning the design and operation of the facility shall be furnished upon request and within the time <br /> frame specified by the EA. <br /> f. Household hazardous waste shall be handled in a manner approved by the LEA. <br /> g. This permit is subject to review by the EA and may be temporarily suspended or revoked at any time by the EA for sufficient <br /> cause, in accordance with Division 30 of the Public Resource Code, Pail 4,Chapter 4,Article 2,Sections 44305 et seq and <br /> associated regulations <br /> h. The facility must conform to the Report of Facility Information dated June 2013 and any subsequent amendments. <br /> i. The EA reserves the right to suspend or modify waste receiving and handling operations due to an emergency,a potential health <br /> hazard, or the creation of a public nuisance. <br /> j. Any change that would cause the design or operation of the facility not to conform to the terms and conditions of this permit is <br /> prohibited. Such a change may be considered a significant change,requiring a permit revision. In no case shall the operator <br /> implement any change without first submitting a written notice of the proposed change,in the form of an RFI amendment,to the <br /> EA at least 180 days in advance of the change. <br /> k. Safety equipment as described in the TPR shall be provided for all employees and workers at the facility. <br /> 1. All waste handling and transfer operations shall take place within the transfer building except for construction and demolition <br /> debris,wood and yard waste composting operations. <br /> m. Personnel working with the"load-screening program"and on the tipping floor shall have training in spill containment,hazardous <br /> waste recognition,personal protective equipment,and the handling of household hazardous materials. <br /> n. An approved hazardous material storage locker shall be provided at the facility for storage of hazardous wastes discovered as a <br /> result of load screening and processing. <br /> o. Specific operational standards which this site must meet are as follows; <br /> p. Litter must be controlled at all times. <br /> q. The windrows will be turned as frequently as necessary to prevent anaerobic degradation and migration of unpleasant odors. <br /> Windrows shall be monitored for internal temperature,spaced and turned appropriately to prevent fires. <br /> r. All composting storage areas/operations cannot contaminate the ground or surface waters,or become and odor nuisance,or <br /> source of vector breeding. <br /> s. The operator shall maintain a log of special/anusual occurrences. This log shall include, but is not limited to,fires, explosions, <br /> the discharge and disposition of hazardous or unpermitted wastes, and significant injuries,accidents or property damage. Each <br /> log entry shall be accompanied by a summary of any actions taken by the operator to mitigate the occurrence. The log shall be <br /> available to site personnel and the EA at all times. <br /> Page 4 of 5 <br />
