CORRRECTIVE ACTION: Provide liner for the biohazardous tote. 24. HSC 118275 -Medical, biohazardous, sharps, trace chemo, pathology, or pharmaceutical waste shall be segregated and contained pursuant to this section. The container shall be properly labeled. Trace chemotherapy waste, as defined in paragraph (5) of subdivision (b) of Section 117690, shall be segregated for storage, and, when placed in a secondary container, that container shall be labeled with the words "Chemotherapy Waste," "CHEMO," or other label approved by the department on the lid and sides, so as to be visible from any lateral direction. OBSERVATION: Chemotherapy containers lack labels OB/GYN room 2009 and Urology room 3219. CORRECTIVE ACTION: Provide the "chemotherapy" labels on all four sides and lid. 25.HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. Biohazardous waste, except as provided in subdivision (b), shall be bagged in accordance with subdivision (b) of Section 118275 and placed for storage, handling, or transport in a rigid container that may be disposable, reusable, or recyclable. Containers shall be leak resistant, have tight-fitting covers, and be kept clean and in good repair. Containers may be recycled with the approval of enforcement agency. Containers may be of any color and shall be labeled with the words "Biohazardous Waste" or with the international biohazard symbol and the word "BIOHAZARD" on the lid and sides so as to be visible from any lateral direction OBSERVATION: Biohazard label is lacking on lid and 2 sides of biohazard container in Urology room 3219. CORRECTIVE ACTION:.Provide label on lid and sides. CORRECTED ONSITE 30. HSC 118310 -Designated accumulation area shall be secured and marked with proper signage. OBSERVATION: Sign on designated accumulation area is in disrepair and not legible. CORRECTIVE ACTION: Provide a sign that is legible. Warning signs shall be readily legible during daylight from a distance of at least 25 feet. Notes: 1. I received a completed MWMP completed during inspection. 2. Stericycle Inc is the medical waste hauler 7876 RA Bridgford Rd. Stockton, CA 95206 3. Pharmaceutical waste and sharps are consolidated into containers that sre labeled "Inceineration Only" 4. Observed biohazard containers that were lacking labels on all four sides and lid during inspection that were corrected onsite. Ensure that containers are labeled properly.
