Per Juan Mora, Jeffrey Helmrich has retired. <br /> -phone numbers for the primary and secondary emergency contacts <br /> REGULATION GUIDANCE: This information must be current and submitted as part of the business plan. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a minor violation. <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The following materials were found on site and have not been reported to CERS: <br /> -one blue plastic 55 gallon drum of hydrochloride was observed in the meat sanitation room <br /> -one white plastic 55 gallon drum of backstop(alcohol based sanitizer)was observed in the meat sanitation room <br /> REGULATION GUIDANCE:Any material that meets or exceeds the reportable quantity shall be reported in the <br /> facility's business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a minor violation. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The map does not address internal roads(if the site does not have <br /> internal roads, it still needs to be documented on the map as"none"). Page 3 has a list of all materials and their <br /> locations.
