DRAFT [TENTATIVE] MONITORING & REPORTING PROGRAM R5-2023-XXXX 44 <br />FORWARD, INC. <br />FORWARD LANDFILL <br />SAN JOAQUIN COUNTY <br />i. A discussion on the results of Annual LCRS Testing conducted in <br />accordance with Section D.1.a. <br />j. Annual updates to the Concentration Limits for all Monitoring <br />Parameters and WQPS Monitoring Points, in accordance with <br />Section B.4.g of this Order. <br />k. To assess the progress of ongoing Corrective Action at the Facility, <br />the following: By 31 July each year, the discharger shall submit an <br />Annual Corrective Action Evaluation Report that includes at least <br />the following: <br />i. A comparison of the site-wide Total VOC (TVOC) mass from <br />the past four quarters to the TVOC mass threshold limit; <br />ii. a screening of the site-wide TVOC mass from the previous four <br />quarters; <br />iii. a comparison of the individual well TVOC concentrations from <br />the previous four quarters to their respective threshold limits; <br />iv. a comparison of the groundwater monitoring results from over <br />time for each COC, for each well; <br />v. quarterly groundwater potentiometric surface maps from the <br />previous year, incorporating data from the DMP, CAP and EMP <br />monitoring wells: <br />vi. a discussion of the landfill cover, including any performance <br />issues from the previous year; <br />vii. a summary and discussion of the previous year’s monitoring <br />data from the entire gas monitoring system and extraction <br />network and; <br />viii. a discussion of overall corrective action progress, including any <br />proposed enhancements to the program. <br />3.Leachate Seep Reporting—Upon discovery of seepage from any <br />disposal area within the Facility, the Discharger shall immediately notify <br />the Central Valley Water Board via telephone or email; and within seven <br />days, submit a written report with the following information: <br />a. Map(s) depicting the location(s) of seepage; <br />b. Estimated flow rate(s);
