OBSERVATION:The designated operator failed to visually inspect the 87 slave spill container for leaks,water, or debris during the 2/23/2023,4/21/2023 and 5/19/2023 visual inspection. REGULATION GUIDANCE: During the inspection, the designated operator shall visually inspect all spill containers, fill pipes,fill caps, and UDC sumps, and any tank top sump that had an alarm for which there is no record of a service visit,for damage, and for the presence of leaks,water, or debris, and check that the sensors are located properly. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are inspecting all of the required areas and noting it on the on the reports. -------------------------------------------------------------------------------------------------------------------------------------------------------- ----------------------------------------------------------------------------- 2). OBSERVATION:The 11/30/2023 designated operator report was signed by the owner/operator on 11/30/2023, the owner/operator failed to provide explanation of follow-up actions in section V that will be taken for the statement provided by the DO in section III. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure to provide follow up actions on compliance issues stated on all designated operator inspection reports . This is a repeat violation, Class II. 214 CCR 2712 Failure to comply with UST operating permit conditions. OBSERVATION: The facility is not in compliance with the written conditions of the operating permit.The owner/operator/contractor Williams Service&repairs Inc replaced the 91 red Jacket main seal O' RING and tested good and checked the 87 slow flow rate on 12/5/2023 without providing notification to EHD. REGULATION GUIDANCE: The owner and operator of underground storage tank(UST)systems shall comply with all requirements of the permit issued for the operation of the UST system. CORRECTIVE ACTION: Immediately take all necessary actions to notify EHD, ust@sjgov.org for all repairs including emergency repairs. This is a minor violation.
