Environmental Health Department <br />CP22 - UST FULL Inspection Report <br />Date: <br />July 29, 2024 <br /> Facility Address: <br /> 5611 E WATERLOO RD, STOCKTON <br /> Facility Name: <br /> BlackHawk Petroleum Inc. <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br />This was corrected on site. <br />This is a repeat violation (Class 2) Violation. <br />CA Code of Regulations 2716(a-e) 2030013 - DO inspections completed at least every 30 days and meet all <br />requirements (TCR 12). <br />OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the <br />8/15/2023 and 1/3/2024 designated operator inspection reports and failed to check that they were responded to <br />appropriately. The missing alarms include: <br />-Sensor out alarm 7/31/2023 <br />-Setup Data Warning alarm 1/3/2024 <br />REGULATION GUIDANCE: During the inspection, the designated operator shall review the alarm history since the <br />previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of the <br />alarm history with documentation taken in response to any alarms to the inspection report. <br />CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the <br />required information on the reports. <br />____________________________________________________________ <br />OBSERVATION: The designated operator failed to check and note all required items on the visual inspection reports. <br />The incorrect and missing information included the following from the inspection reports: <br />-Checking the box to indicate that Floats and Chains are installed in the UDCs on multiple DO Visual Inspection <br />Reports <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required testing <br />and maintenance for the UST system have been completed and shall check all required items on the report . <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all <br />of the required information on the reports. <br />This is a repeat violation (Class 2) Violation. <br /> 56 <br />CA Code of Regulations 2712(b)(1-2) 2030001 - Records of release detection monitoring and maintenance properly <br />maintained (TCR 9d). <br />OBSERVATION: Maintenance and monitoring records for the last three years were not found on site. <br />-Monitoring System Certification results for 7/21/2021 <br />-Monitoring System Certification results for 7/19/2023 <br />-Work order for 1/3/2024 <br />REGULATION GUIDANCE: These records shall be maintained on site for at least three years. Monitoring records <br />include: (1) date and time of all monitoring or sampling; (2) monitoring equipment calibration and maintenance records; <br />(3) results of any visual observations; (4) results of sample analysis performed a lab or in the field; (5) logs of all <br />readings of gauges or other monitoring equipment, ground water elevations, or other test results; (6) results of <br />inventory readings and reconciliations. <br /> 59 <br />FA0002160 PR0231764 SC001 07/29/2024 <br />EHD Rev. 12/06/2021 CP22 - UST FULL OIRPage 6 of 7 <br />1868 E.
