NO. <br />EM20114653 - JWM <br />20-4337 <br />Mr. Jason Gaitan: <br />Please accept this document as the final report detailing the emergency response and remedial actions <br />taken for the diesel fuel release that occurred on 8/13/2020, at the above referenced location. <br />INCIDENT BACKGROUND: <br />On August 13, 2020, at approximately 5:00 a.m. CDT, a tractor -trailer was leaving the above -referenced <br />facility owned by KeHE Distributors, Inc. (KeHE) when the unit struck road debris, damaging the tractor's <br />saddle tank. The driver then turned around and entered the facility owned by KeHE. As a result, <br />approximately 50 gallons of diesel fuel was released to the asphalt and concrete surfaces of the parking lot <br />and two (2) small soil surfaces adjacent to the parking lot. <br />EMERGENCY RESPONSE: <br />On August 13, 2020, at approximately 8:44 a.m. CDT, a representative from KeHE, Mr. Bart Moury, retained <br />Cura Emergency Services, L.C. (CES) to manage the environmental remediation of the site on their behalf. <br />Based on the available information, the CES incident manager dispatched a crew from Ancon Services (AS) <br />to assess and remediate the site as necessary. <br />REGULATORY NOTIFICATION: <br />Pursuant to California state regulations, petroleum related releases in excess of 42 gallons are considered <br />reportable. This release was estimated to be approximately 50 gallons of diesel fuel; therefore, regulatory <br />notification was necessary. <br />On August 13, 2020, at approximately 1:32 p.m. CDT, the CES incident manager verbally reported the <br />incident to the California Office of Emergency Services (CAGES) hotline on behalf of KeHE. The duty officer <br />for CAOES issued incident number 20-4337 to the incident. At approximately 1:42 p.m. CDT, the CES <br />incident manager further reported the incident to the San Joaquin County Environmental Health (SJCEH) <br />REMEDIAL ACTIONS: <br />On August 13, 2020, at approximately 10:53 a.m. CDT, a crew from AS arrived onsite.
