SAID dOAQUIN <br />COUNTY <br />Environmental Health Department <br />CP22 - UST FULL Inspection Report <br />Facility Name: <br />Facility Address: <br />Date: <br />GAVINKO PETROLEUM* <br />7700 MORELAND ST, STOCKTON <br />October 29 2024 <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />Item # <br />Remarks <br />REGULATION GUIDANCE: A copy of the inspection report form must be submitted within 30 days of the test. <br />CORRECTIVE ACTION: Immediately provide this report to the EHD. <br />This is (Minor) Violation. <br />47 <br />CA Health and Safety Code 25290.1(c3), 25290.2(c3) 2030008 - Secondary containment installed after July 1, 2003 is <br />free of water. <br />OBSERVATION: Approximately 1 cup of water was found in the following sumps: <br />- Diesel STP sump <br />- 91 Fill sump <br />- 91 STP sump <br />REGULATION GUIDANCE: Secondary containment shall be constructed to prevent any water intrusion into the <br />system by precipitation, infiltration, or surface runoff. <br />CORRECTIVE ACTION: Immediately remove this liquid, make a hazardous waste determination per Title 22 hazardous <br />waste regulations, and manage accordingly. Immediately contact a properly licensed, trained, and certified contractor <br />to address the water intrusion into the Diesel STP sump, 91 Fill sump, 91 STP sump under permit and inspection of <br />the EHD, if needed. <br />This is a repeat violation (Class 2) Violation. <br />51 <br />CA Health and Safety Code 25292.1(a) 2030035 - UST system is operated to prevent unauthorized release, including <br />spills and overfills. <br />OBSERVATION: Alarms were not responded to or addressed for the Designated Operator (DO) reports for the last 14 <br />months, no work orders were found on site and in DO reports the Owner did not address the compliance issues when <br />they were mentioned by the DO. <br />REGULATION GUIDANCE: The underground storage tank system shall be operated to prevent unauthorized releases, <br />including spills and overfills. <br />CORRECTIVE ACTION: Ensure all future alarms and potential releases are responded to immediately as needed. <br />This is a repeat violation (Class 2) Violation. <br />53 <br />CA Health and Safety Code 25292.2; CCR 2711(a)(11) 2010007 - Submitted/maintained current Cert of Financial <br />Responsibility/mech of financial assurance (TCR 11). <br />OBSERVATION: Financial responsibility documents are incomplete <br />- Sections B & E of Certificate of Financial Responsibility are NOT complete <br />REGULATION GUIDANCE: Current and complete financial responsibility documents are required to be submitted <br />annually. <br />CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS) at <br />http://cers.calepa.ca.gov/, and upload the required documents. <br />EHD Rev. 12/06/2021 <br />FA0003732 PR0231819 SCO01 10/29/2024 <br />Page 7 of 10 CP22 - UST FULL OIR <br />1868 E.
