Turner Rd., Lodi <br /> The following requirements have been identified as pertinent to this project. Other requirements may also <br /> apply. These requirements cannot be modified. <br /> 1. Submit Water Provision Declaration form to the Environmental Health Department for review. <br /> 2. Should the number of non-resident individuals exceed 24 for at least 60 days per year, or the <br /> number of service connections exceed four, a yearly permit to operate a public water system will <br /> be required by the Environmental Health Department (San Joaquin County Development Title, <br /> Section 9-602.010 and 9-601.030.). <br /> The supplier must possess adequate financial, managerial, and technical capability to assure <br /> delivery of pure, wholesome, and potable drinking water in accordance with San Joaquin County <br /> Development Title, Sections 9-602.010 and 9-601.030 and C.C.R., Title 22, and Health and <br /> Safety Code, Section 116525 through 116570. <br /> 3. Submit two (2) hardcopy sets, or one (1) electronic version, of food facility plans to the <br /> Environmental Health Department for review and approval prior to issuance of building permit(s) <br /> (California Retail Food Code, Article 1, 114380). Grease trap may be required depending on type <br /> of food prepared onsite. The fee will be based on the current schedule at the time of payment. <br /> Applicant may contact Jeff Carruesco, Program Coordinator, Food and Restaurants Program, at <br /> (209) 468-3438 for any questions. <br /> 4. Before any hazardous materials/waste can be stored or used onsite, the owner/operator must <br /> report the use or storage of these hazardous materials to the California Environmental Reporting <br /> System (CERS) at cers.calepa.ca.gov/and comply with the laws and regulations for the programs <br /> listed below (based on quantity of hazardous material in some cases). The applicant may contact <br /> the Program Coordinator of the CUPA program, Elena Manzo (209) 953-7699, with any questions. <br /> 1.
