SAN I-J O A Q U IN Environmental Health Department <br /> CUUN1 Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> STOCKTON PORT DIST(STOCKTON, CA) 120 Hooper Dr, Stockton December 07, 2023 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 11 HSC 25505(a)(4)Failed to provide or document initial and/or refresher training to appropriate personnel. <br /> OBSERVATION: Employees training records were not available for inspection. <br /> REGULATION GUIDANCE: The business plan shall include provisions for ensuring that appropriate personnel <br /> receive initial and annual refresher training.All employees shall be trained in safety procedures in the event of a <br /> release or threatened release of a hazardous material, including, but not limited to,familiarity with the following <br /> plans and procedures: <br /> (A)Immediate notification contacts to the appropriate local emergency response personnel and to the unified <br /> program agency. <br /> (B)Procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment. <br /> (C) Evacuation plans and procedures, including immediate notice,for the business site. <br /> This training shall be documented electronically or by hard copy and shall be made available for a minimum of three <br /> years. <br /> CORRECTIVE ACTION: Immediately provide employee training for appropriate personnel and submit a copy of the <br /> training records to the EHD. <br /> This is a Class II violation. <br /> 12 HSC 25505(c) Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Employees do not have access to the Emergency Response Plan and the inventory information <br /> on-site. <br /> REGULATION GUIDANCE:The emergency response plans and procedures,the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were two violations observed during <br /> the inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a <br /> statement documenting the corrective actions that have been or will be taken for each violation, and any <br /> supporting paperwork, by 01/06/2024. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($162).
