The secondary containment date was cited on 1/17/2023 on the designated operator inspection reports dated 3/11/2023,4/10/2023, and 7/6/2023.The correct date for the secondary containment test is 1/13/2021. The monitoring system certification date was missing from the designated operator inspection report for 6/8/2023. The correct date for the monitoring system certification test is 1/13/2023. REGULATION GUIDANCE: During the inspection, the designated operator shall check that all required testing and maintenance for the UST system have been completed and document the dates they were done. CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of the required information on the reports. #2 OBSERVATION: Designated operator reports were not signed within the required time frame. -The 1/13/2023, 2/10/2023, 3/11/2023, and 7/6/2023 designated operator report did not include the printed name of the owner/operator.. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This is a repeat violation, Class II. 215 CCR 2715 Designated operator, service tech, or installer did not possess current certifications. OBSERVATION: Felix Ramirez performed the 4/10/2023 inspections and did not have a current ICC certification at the time of the inspections. REGULATION GUIDANCE:A designated operator shall have a current ICC certification at the time of the inspections. CORRECTIVE ACTION: Ensure that any designated operator who performs inspections at this facility has current ICC certification, and submit documentation to the EHD showing that Felix Ramirez had current ICC certification when the 4/10/2023 inspections were performed.
