S A N J D AR I N Environmental Health Department <br /> lj ;'�J1Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> A&A GAS&FOOD MART 16 E HARDING WAY, STOCKTON February 05, 2024 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The Hazardous Materials Inventory forms for gasoline were not complete.The gasoline incorrectly <br /> lists 25,000 gallons maximum daily but there are 20,000 gallon and 12,000 gallon tanks on site equaling 32,000 <br /> gallon maximum. <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The following were missing on the site map: <br /> -evacuation staging area <br /> -definition of"HZM" <br /> -two fire extinguishers observed by the entrance of the store and in the back storage near the absorbent <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER)procedures were lacking or inadequate.
