S A N J D AR I N Environmental Health Department <br /> lj ;'�J1Y <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> A&A GAS&FOOD MART 16 E HARDING WAY, STOCKTON February 05, 2024 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 5 HSC 25505(a)(1) Failed to electronically submit hazardous materials inventory for all reportable materials on site. <br /> OBSERVATION: The Hazardous Materials Inventory forms for gasoline were not complete.The gasoline incorrectly <br /> lists 25,000 gallons maximum daily but there are 20,000 gallon and 12,000 gallon tanks on site equaling 32,000 <br /> gallon maximum. <br /> REGULATION GUIDANCE :The Hazardous materials inventory information must be current and submitted as part <br /> of the business plan. <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The following were missing on the site map: <br /> -evacuation staging area <br /> -definition of"HZM" <br /> -two fire extinguishers observed by the entrance of the store and in the back storage near the absorbent <br /> REGULATION GUIDANCE:A site map shall contain a north orientation, loading areas, internal roads, adjacent <br /> streets, storm and sewer drains, access and exit points, emergency shutoffs, evacuation staging areas, hazardous <br /> material handling and storage areas, and emergency response equipment. <br /> If a site map element is not applicable for your facility then list it on the map and label as"NA". <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 7 HSC 25505(a)(3) Failed to electronically submit an adequate ER procedures for a release or threatened release. <br /> OBSERVATION: The emergency response(ER)procedures were lacking or inadequate. The primary and <br /> secondary emergency contact persons could not be reached at their 24 hour phone numbers listed in CERS and in <br /> the Consolidated Emergency Response/Contingency Plan during the inspection. <br /> REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a <br /> release or threatened release of hazardous materials, including, but not limited to,the following: <br /> -immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice,for the site <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a Class II violation. <br /> FA0003610 PR0521021 SCO01 02/05/2024 <br /> EHD 19-01 Rev.12/06/2021 Page 2 of 4 Hazardous Materials Program OIR <br /> 1868 E.
