The following is an itemized list of underground storage tank violations that have not <br /> been addressed for CHARTER WAY 76 as of February 02, 2024. <br /> Open violations from December 27, 2023 inspection <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Violation#208-Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION: The designated operator failed to document all the alarms from the attached alarm history on the <br /> May 16, 2023 designated operator inspection report and failed to check that they were responded to appropriately. <br /> The missing alarms include: L13-diesel fill sump brine sensor. <br /> REGULATION GUIDANCE: During the inspection,the designated operator shall review the alarm history since the <br /> previous inspection, check that each alarm was documented and responded to appropriately, and attach a copy of <br /> the alarm history with documentation taken in response to any alarms to the inspection report. <br /> CORRECTIVE ACTION: Ensure that designated operators performing inspections at this facility are including all of <br /> the required information on the reports. <br /> -------------------------------------------------- <br /> OBSERVATION: Corrective action was not documented on the following DO(Designated Operator)reports. <br /> -March 21, 2023 <br /> -April 18,2023 <br /> -May 16, 2023 <br /> -July 10, 2023 <br /> -September 6, 2023 <br /> -October 4, 2023. <br /> -October 31, 2023. <br /> -November 29, 2023. <br /> REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, <br /> the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 <br /> hours of being provided a signed copy of the report,the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance <br /> issues. <br /> ❑ This violation was corrected ❑This violation will be corrected by(date): <br /> ❑ Supporting documents included <br /> Describe actions taken or will be taken to correct violation: <br /> Page 3 of 4 <br />
