Environmental Health Department <br />Underground Storage Tank Program Inspection Report <br />Date: <br />February 15, 2024 <br /> Facility Address: <br /> 16500 S HARLAN RD, LATHROP <br /> Facility Name: <br /> LATHROP SHELL <br />SUMMARY OF VIOLATIONS <br />(CLASS I, CLASS II, or MINOR - Notice to Comply) <br />RemarksItem # <br /> 208 CCR 2716(a-e) Failure to comply with the designated operator inspection requirements. <br />OBSERVATION: The designated operator failed to check and note all required items on the visual inspection <br />reports. The incorrect and missing information included the following from the inspection reports: <br />-Section 6, Printed name of UST Owner/Operator <br />-Section 9, Indication of mechanical float mechanisms used in UDCs <br />-Section 10, Accurate date last performed for secondary containment testing <br />REGULATION GUIDANCE: During the visual inspection, the designated operator shall check that all required <br />testing and maintenance for the UST system have been completed and shall check all required items on the report. <br />CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including <br />all of the required information on the reports. <br />This is a repeat violation, Class II. <br /> 308 CCR 2632(c), 2634(d), 2636(f) Failure of the leak detection equipment to have an audible and visual alarm. <br />OBSERVATION: The North BRAVO FLOAT AND CHAIN in the 3/4 failed to stop the flow of product at the <br />dispenser when tested. <br />REGULATION GUIDANCE: All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. <br />CORRECTIVE ACTION: Correct immediately by having a properly licensed, trained, and certified contractor replace <br />the failed component with a functional component (LG 113-x listed, if applicable), and obtain a permit within one <br />business day from the EHD, if needed. If the failed component can’t be replaced immediately, there is a possibility <br />that the UST system may be red tagged to prevent fuel inputs. <br />-------- <br />OBSERVATION: The float and chain in the 1/2 failed to stop the flow of product at the dispenser when tested. <br />REGULATION GUIDANCE: All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. <br />CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. <br />This is a Class II violation. <br />Overall Inspection Comments: <br />An inspection checklist was provided to the facility operator on the day of inspection.
