The most recent permit on site <br /> was the 2022 operating permit. <br /> REGULATION GUIDANCE:A copy of the UST operating permit and all attachments and conditions shall be <br /> retained at the facility. <br /> CORRECTIVE ACTION:This was corrected on site by obtaining a copy of the 2023 operating permit from the EHD <br /> website. <br /> This is a repeat violation,Class 11. <br /> 208 CCR 2716(a-e)Failure to comply with the designated operator inspection requirements. <br /> OBSERVATION:Designated operator reports were not signed within the required time frame. <br /> -The 3/28/2023 designated operator report was not signed by the owner/operator. <br /> REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection, <br /> the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 <br /> hours of being provided a signed copy of the report,the owner or operator shall provide a description of each <br /> corrective action taken or to be taken and shall sign and date the report,acknowledging the identified compliance <br /> issues. <br /> CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the <br /> required time frame in the future. <br /> This is a repeat violation,Class II. <br /> 305 HSC 25290.1,25290.2,25291 Failed to maintain secondary containment(e.g.failure of secondary containment <br /> testing). <br /> OBSERVATION:The following were observed indicating a leak in the secondary containment: <br /> -Brine was observed in the 91 STP sump,Diesel STP sump,Diesel fill sump <br /> -Liquid in the 87 fill sump <br /> -Brine in the 15/16 UDC <br /> REGULATION GUIDANCE:Secondary containment shall be impervious to the liquid and vapor of the substance <br /> contained and constructed to prevent structural weakening as a result of contact with any hazardous substances <br /> released from the primary containment. <br /> CORRECTIVE ACTION:Immediately contact a properly licensed,trained,and certified contractor to make repairs <br /> to the UST system under permit and inspection of the EHD. <br /> This is a repeat violation,Class 11. <br /> FA0024482 PRO542573 SC001 12/20/2023 <br /> EHD 23.01 Rev,12/06/2021 Page 5 of 6 Underground Storage Tank Program OIR <br /> 1868 E.
