Each sterilization load shall be monitored with mechanical indicators for time, temperature and pressure, Sterilization shall be <br />aocomplished by steam autoclave, manufactured for the sterilization of medical instruments, and pass a Class V ntegrator test. <br />HSC 119315(b) — EASYCLAVE does not have a pressure gauge. Ensure that sterilizer has indicators for tine, <br />temperature, and pressure and is approved by EHD prior to use. <br />v/4. Sterilizer shall be spore -tested after initial installation, after major repair, and at least once per month. A writer sterilization <br />shall be maintained for 3 years. The log shall include spore -test results each sterilization cycle, date, contents, exposure time and <br />temperature, results of the Class V integrator for every cycle/load, and evidence of an acceptable spore test bEfere reuse of the <br />sterilizer after a failed spore -test. HSC 119315(b) — Time recorded on sterilization log does not show actual time when <br />instruments are being sterilized. Ensure that the log is accurate according to the time when the instruments are being <br />sterilized. <br />5. Deco ntaminaiion and sterilization areas shall be separated from procedure areas by a space of at least 5 feet or by a <br />cleanable barrier and equipped with a sink with hot and cold running water, containerized liquid soap, and single L se paper <br />towels dispensed from a wall -mounted, touchless dispenser that is readily accessible to practitioner. HSC 119314(c) -Cleaning <br />supply was observed in sink area of sterilization room. Ensure that cleaning supplies is stored in a cor to ner away from <br />the sterilization area. v0 ► IRV <br />6. A facility that uses single use instruments shall maintain record of purchase, log of procedures, names of prac-itioner and <br />client, da -.e of procedure, instruments used, and written evidence om the manufacture that the instruments have undergone <br />sterilization. The record of purchase, evidence of sterilization, ty e and number of instruments used shall be m 3irtained fc - a <br />minimum of 90 days after use. HSC 119315(f) — The invoice and sterilization certificate for instruments PEAK CARTRIDGE <br />LOT #202106 were not available. Provide EHD with the invoice and sterilization certificates for the instruments. <br />7. The sharps waste container shall be within arm's reach and labeled with the word "sharps waste" or with the biohazard symbol <br />and the word "Biohazard". Sharp waste containers shall be disposed by a licensed waste hauler or approved mai back system. <br />Vume-tation of proper disposal shall be maintained for 3 years. HSC 119314(e) — Sharps waste container was not within <br />'s reach of procedure table. Ensure that sharps waste container is within arm's reach of the procedure table a rid <br />that the procedure table is 5 feet apart from each other. <br />32. A body art facility shall have floors, walls, and ceilings that are smooth, nonabsorbent, free of open holes, anc washat. e. <br />The body art facility shall be free of insect and rodent infestation. The procedure area must be separated, by a wall, ceilinc to <br />floor, from nail ant hair activities and be separated from all business not related to body art. Approval of co -located body art <br />facilities are at the discretion of the LEA. A procedure area must be equipped with adequate light to perform a orocedure. Each <br />procedure area shall have lined waste containers. HSC 119314 & 119317 —Flooring had spots that were not painted. 'Nall <br />oVets have openings. Ensure that the flooring is painted and is nonabsorbent and all openings near the wall out ets <br />have to be patched/repaired. <br />Reinspection on/abort: A reinspection fee of $156 per hour may be charged.
