This notice is issued pursuant to Title 27, California <br />Code of Regulations, Section 21675: <br />The Solid Waste Facility Permit issued to Forward Resource Recovery on May 3, 2004 is due for permit review <br />on May 3, 2014. <br />Please submit an application for a permit review (attached) (form CIWMB E-1-77) to the San Joaquin County <br />Environmental Health Department (EHD) within 30 days of receipt of this letter. Submit a plan review fee of $625 <br />with the review application. Submit a Service Request (SR) form (attached). Upon completion of the review, the <br />EHD will determine if a revision of the existing SWFP is required. <br />The EHD understands that you may want to revise the SWFP. This process can be done congruently with the five- <br />year permit review. The complete application package submitted for the 5 year review would need to include the <br />proposed revisions. The operator requirements for submittal of an application are described in Title 27 Section <br />21640.
