The report documents the <br /> construction and construction quality assurance (CQA)procedures for the northeast portion of Class HI <br /> Waste Management Unit(WMU)FU-04. The report documents construction and quality assurance <br /> activities associated with: construction of a perimeter berm with 60-mil HDPE Geomembrane, <br /> preparation of sub-grade, installation of placement of an 8-oz geotextile, overlain with a minimum 6- <br /> inch thick gravel blanket LCRS, overlain with a second 8-ox geotextile. In addition, 1-foot protective <br /> operations layer was installed over the LCRS. <br /> The report as presented is incomplete. The Final Construction Quality Assurance Report for a waste <br /> management unit must include sufficient information for RWQCB staff to approve the construction. <br /> The report provides daily construction reports, CQA test data and sketch maps of where some of the <br /> tests were conducted. The report does not discuss layout of the perimeter berm, layout of the 3-inch <br /> drainage pipe, the wrapped coarse gravel, or the system to convey leachate off the WMU. These are <br /> important parts of the approved design and there is no documentation that they were constructed as <br /> designed. Additionally, adequate final construction documentation(As-Built drawings) are not <br /> provided, the one As-Built drawing provided sketches an outline, which appears to be WMU,with <br /> apparent survey stations and elevations of either top of sub-grade; top of gravel; or top of operations <br /> layer with no explanations. At a minimum the report must provide: As Built Drawings for the Sub- <br /> grade; the test locations and the final grade should be submitted. At least one of the plans should <br /> indicate: the drainage system including pipe layout, connection to sump and limits of coarse gravel. <br /> The As Built Drawings contained in Revision I of Final Report for Waste Management Unit ART-03, <br /> prepared by Vector Engineering Inc.
