(Closed on 8/17/2023 RTC) #113: Failure to conduct secondary containment testing. • CORRECTIVE ACTION: Provide a statement that ensure your facility will conduct the secondary containment testing after works are completed for service requests. • The Secondary Containment testing conducted on 07/13/2023 is not accepted due to EHD did not receive a testing notification.A 48 hours notification is required. • A retest of the Secondary Containment is needed to finalize the service request. #120: Failure to submit and/or maintain documentation regarding compatibility for UST system components. • CORRECTIVE ACTION: Immediately, submit an affirmative statement of compatibility or replace the Red Jacket FXIV with a component that meets all applicable Title 23 requirements for a UST system. • 1/3/2023 emailed documents is not consist of the Red Jacket FXIV you had included the Fe Petro. Please submit an updated scope of work, parts list, cut sheet for the Red Jacket leak detector, and certification for the installer who did the work.
