(Closed on 8/17/2023 RTC) <br /> #113: Failure to conduct secondary containment testing. <br /> • CORRECTIVE ACTION: Provide a statement that ensure your facility will conduct the secondary containment <br /> testing after works are completed for service requests. <br /> • The Secondary Containment testing conducted on 07/13/2023 is not accepted due to EHD did not receive a <br /> testing notification.A 48 hours notification is required. <br /> • A retest of the Secondary Containment is needed to finalize the service request. <br /> #120: Failure to submit and/or maintain documentation regarding compatibility for UST system components. <br /> • CORRECTIVE ACTION: Immediately, submit an affirmative statement of compatibility or replace the Red Jacket <br /> FXIV with a component that meets all applicable Title 23 requirements for a UST system. <br /> • 1/3/2023 emailed documents is not consist of the Red Jacket FXIV you had included the Fe Petro.
