OBSERVATION:Annual monitoring system certification and leak detector testing was performed on 5/25/2023 and a test report was submitted with correct leak detector listed on 4/15/2024, 295 days late. Test results was submitted on 6/21/2023 but was not accepted due to incorrect leak detector listed. REGULATION GUIDANCE:A copy of the test report must be submitted within 30 days of the tests. CORRECTIVE ACTION: Take all necessary precautions to ensure testing and submittal of test reports are performed in a timely manner. OBSERVATION: Designated operator reports were not signed within the required time frame. Multiple designated operator reports were not signed within 72 hours by the owner/operator. Per Tara Hickman,those were dates she was out of the office. Discussed if it was possible to have an alternate review reports when she is out. REGULATORY GUIDANCE:Within 48 hours of the completion of the designated UST operator visual inspection,the designated operator shall sign and provide the owner or operator with a copy of the inspection report. Within 72 hours of being provided a signed copy of the report,the owner or operator shall provide a description of each corrective action taken or to be taken and shall sign and date the report, acknowledging the identified compliance issues. CORRECTIVE ACTION: Ensure all designated operator inspection reports are completed and signed within the required time frame in the future. This information was required to be submitted by 8/12/2023.This information was received on 8/17/2023, 5 days late. REGULATION GUIDANCE:An operator that receives an inspection report shall have 30 days to implement the corrections specified in the inspection report and comply with permit conditions.
