The monitoring plan was <br /> rejected for the following: <br /> -The Financial Responsibility Mechanisms under UST Facility Operating Permit Application has"Other <br /> Mechanisms"selected but no indication of what the other mechanisms are. Only CFO and State Fund are listed on <br /> the Certification of Financial Responsibility. <br /> -The Certification of Financial Responsibility uploaded does not have Section B completed with tank owner or <br /> operator name <br /> -The UST Owner/Operator: Written Agreement is marked as exempt but does not provide a statement that explains <br /> way the facility is exempt or why an agreement is not needed. <br /> REGULATION GUIDANCE: The monitoring plan must be uploaded to the California Environmental Reporting <br /> System (CERS). <br /> CORRECTIVE ACTION: Immediately log into CERS, make the necessary changes, and submit for review by the <br /> EHD. <br /> This is a repeat violation, Class II. <br /> 112 CCR 2638, 26410), 2715(f) Failure to have properly qualified service technician test leak detection equipment every <br /> 12 months. <br /> OBSERVATION:Annual monitoring system certification and leak detector testing were previously performed on <br /> 3/21/2023 and was completed on 4/8/2024, 1 months past due. <br /> REGULATION GUIDANCE: These tests are required once every 12 months. <br /> CORRECTIVE ACTION: Take all necessary precautions to ensure testing is performed in a timely manner. <br /> This is a Class II violation. <br /> Overall Inspection Comments: <br /> FA0003679 PR0232555 SCO01 04/08/2024 <br /> EHD 23-01 Rev.12/06/2021 Page 4 of 5 Underground Storage Tank Program OIR <br /> 1868 E.
