Please fill out and submit the enclosed application and current <br /> fee payment to the EHD at least 45 days prior to the planned date of occupancy of your facility. <br /> Please note that a permit to operate is required only for employee housing facilities that house <br /> five (5) or more employees at a location (not including relatives) with accommodations that may <br /> include manufactured homes, single family residences, recreation vehicles, multi-family dwelling <br /> units, or any other living quarters or accommodations, and the premises/property upon which they <br /> are situated. <br /> If you do not intend to use the facility this year, or if the employee housing facility houses less <br /> than 5 employees, please complete and return the enclosed "Certificate of Non-Operation". <br /> If there are any changes to the owner and/or operator information, a "Statement of Citizenship, <br /> Alienage and Immigration Status in Support of a Request for State Public Benefits" form must be <br /> completed and submitted to the Environmental Health Department prior to the issuance of a <br /> permit to operate. Please contact Genesis Martinez at (209) 468-0217 or gmartinez@sjgov.org <br /> to obtain this form. <br /> For your reference, the laws and regulations for employee housing facilities may be found in the <br /> California Health and Safety Code, Division 13, Part 1, and in the California Code of Regulations, <br /> Title 25, Chapter 1, Subchapter 3, established by the Employee Housing Act. <br /> Please return the completed application form and fee payments, or the Certification of Non- <br /> Operation, in the enclosed envelope.
