The attached Annual Permit Application Package was mailed to you on <br /> October 16, 2025. However, no submittals have been received. <br /> Please fill out and submit the enclosed application and current fee payment at least 45 days prior <br /> to the planned date of occupancy of your facility. <br /> Please be advised that failure to submit the required application and fee payments at least 45 <br /> days prior to the date of initial occupancy and operating without a permit is a violation of <br /> California Health and Safety Code, Section 17032, and 17030(a), respectively. <br /> If you do not intend to use the facility this year, or if the employee housing facility houses less <br /> than 5 employees, please complete and return the enclosed "Certificate of Non-Operation". <br /> Once the permit application and fee is processed, the EHD will contact you to schedule an onsite <br /> inspection of the employee housing facility. <br /> Failure to comply with the permitting requirements may result in further enforcement action <br /> billable at the current hourly rate. <br /> If you have any questions, please contact Frank Girardi Lead Senior REHS, at (209) 953-6213 <br /> or fvgirardi@sjgov.org. <br /> Enclosures <br /> 1868 E.
