A N J O A Q U I Environmental Health Department <br /> COUNTY— <br /> Hazardous Materials Program Inspection Report <br /> Facility Name: Facility Address: Date: <br /> ALL COUNTIES GLASS 2020 MINER AVE, STOCKTON April 04, 2024 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> 10 HSC 25508.1(a)-(f) Failed to electronically update the HMBP within 30 days of substantial change in operations. <br /> OBSERVATION: During the inspection, Jason Minjarez stated that Dane Treiber and Ben Alarca have not been with <br /> the business since 2023. Both Dane and Ben were listed as the primary and secondary emergency contacts. The <br /> business has not updated and submitted the information within 30 days. <br /> REGULATION GUIDANCE: Within 30 days of one the following events, business shall electronically update the <br /> information submitted to the California Environmental Reporting System (CERS): <br /> a) A 100 percent or more increase in the quantity of a previously disclosed material. <br /> b) Any handling of a previously undisclosed hazardous materials <br /> c) A change of business or facility address <br /> d)Change business ownership <br /> e)Change of business name <br /> f)(1)A substantial change in the handler's operations occurs that requires modification to any portion of the <br /> business plan. <br /> (2)For the purpose of this subdivision, "substantial change" means any change in a facility that would inhibit <br /> immediate response during an emergency by either site personnel or emergency response personnel, or that could <br /> inhibit the handler's ability to comply with Section 25507 , change the operational knowledge of the facility,or <br /> impede implementation of the business plan. <br /> CORRECTIVE ACTION: This was corrected on site by logging into the California Environmental Reporting System <br /> (CERS)at http://cers.calepa.ca.gov/, entering the correct or updated information, and submitting to the EHD for <br /> approval. <br /> This is a Class II violation. <br /> 12 HSC 25505(c)Failed to have the HMBP readily available to facility or EHD personnel. <br /> OBSERVATION: Facility could not provide the Emergency Response Plan and the inventory information upon <br /> request. Employees do not have access to the Emergency Response Plan and the inventory information on-site. <br /> REGULATION GUIDANCE: The emergency response plans and procedures,the inventory of information required <br /> by this article, and the site map required by this section shall be readily available to personnel of the business or the <br /> unified program facility with responsibilities for emergency response or training pursuant to this section. <br /> CORRECTIVE ACTION: Immediately make arrangements to have the business plan readily available for review. <br /> This is a minor violation. <br /> Overall Inspection Comments: <br /> Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were six violations during the <br /> inspection. Five violations were corrected on site. Complete and submit a copy of the Return to Compliance <br /> Certification form to the EHD with a statement documenting the corrective actions that have been or will be <br /> taken for each violation, and any supporting paperwork, by 5/4/2024. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign.
