OBSERVATION:A new designated operator, Genardo Perez, began conducting inspections on 9/8/2023 and notification was not provided to the EHD within 30 days of the change. REGULATION GUIDANCE:The owner or operator shall submit a"Designated Underground Storage Tank Operator Identification Form" no later than 30 days after any change in the designated UST operator(s). CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov and upload the notification identifying all the designated operators for this facility. This is a repeat violation (Class 2) Violation. 56 CA Code of Regulations 2716(a-e)2030013-DO inspections completed at least every 30 days and meet all requirements(TCR 12). OBSERVATION:The designated operator failed to check and note all required items on the visual inspection reports. The incorrect and missing information included the following from the inspection reports: -6/21/2023 section#6 did not include a printed name -9/8/2023 DO report is missing section#6 signature, missing CERS ID, and date -All the DO reports contained the wrong Secondary Containment date of 12/29/2021. REGULATION GUIDANCE: During the visual inspection,the designated operator shall check that all required testing and maintenance for the UST system have been completed and shall check all required items on the report. CORRECTIVE ACTION: Ensure that designated operators performing visual inspections at this facility are including all of the required information on the reports.
