Multiple designated <br /> operator reports have listed that Designated Operator Training needs to be conducted. The training on site was last <br /> dated 1/19/2023. <br /> REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and <br /> maintenance of the UST system at least once every 12 months. The training shall be conducted before the <br /> individual performs the duties of a facility employees.The training shall include, but is not limited to: <br /> 1. Operation of the UST system in a manner consistent with the facility's best management practices <br /> 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan <br /> 3. Employee's role with regard to spills and overfills as specified in the facility's response plan <br /> 4. Name of the contact person(s)for emergencies and monitoring equipment alarms <br /> Initial training shall also include a site-specific practical demonstration that provides both sharing information and <br /> evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least <br /> one of the facility employees present during operating hours shall have current training. <br /> CORRECTIVE ACTION: Ensure that employees have been trained by the designated operator, maintain the list on <br /> site, and submit a copy of the training records to the EHD. <br /> This is a repeat violation, Class II. <br /> 321 HSC 25299(a)(9)Leak detection equipment disabled or tampered with. <br /> OBSERVATION: Designated operator report and work order dated 12/31/2023 stated "L1 fuel alarm occurred due <br /> to water entering the Diesel STP sump. Site lifted sensor, clearing the alarm.Water needs to be removed and <br /> sensor needs to be lowered to lowest point of sump" <br /> REGULATION GUIDANCE:An operator of an underground storage tank system shall not allow tampering with or <br /> otherwise disabling automatic leak detection devices or alarms. <br /> CORRECTIVE ACTION: Immediately maintain all sensors at the lowest point of sumps and maintain the monitoring <br /> system in proper operating conditions at all times. <br /> This is a Class I violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 5/30/2024. <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead <br /> of having them sign. Starting September 1, 2018, all in-office CERS help will be provided at EHD hourly rate <br /> ($162).
