OBSERVATION: The emergency response(ER)procedures were not complete. The hospital/medical facility name and phone number was not provided on lines C6&C7. Section G is missing the locations for fire alarm boxes and telephones on line G60 and G30. REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a release or threatened release of hazardous materials, including, but not limited to,the following: -immediate notification of local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to persons, property, or the environment -evacuation plans and procedures, including immediate notice,for the site CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. This is a repeat violation, Class II. 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. OBSERVATION: The business plan information has not been reviewed and resubmitted in the California Environmental Reporting System (CERS)annually by the due date, January 15.The business plan was submitted late on 6/12/2018 for the year of 2018.The business plan was not submitted for the year of 2019.The business plan wa submitted late on 2/27/2020 for the year of 2020.The business plan was submitted late on 5/13/2022 for the year of 2022.The business plan was submitted late on 8/25/2023 for the year 2023.Th business plan was submitted on time for 2024. This violation has been corrected. REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar year and may be submitted beginning November 1 of the previous year. CORRECTIVE ACTION: Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. This is a minor violation. Overall Inspection Comments: Conducted a Hazardous Materials Business Plan (HMBP) inspection. There were were 2 violations found during the inspection. Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, by 5/9/2024. To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of having them sign.
