Thirteen 5 gallon plastic containers of Flo-Klor were observed in the outdoor chemical storage <br /> 2. Four 55 gallon drums of glycol were observed near the south fence <br /> CORRECTIVE ACTIONS: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and submit to the EHD for <br /> approval. <br /> This is a repeat violation, Class II. <br /> 6 HSC 25505(a)(2) Failed to electronically submit a site map with all required content. <br /> OBSERVATION: The site map is incomplete. The map is missing the following: <br /> 1. loading areas <br /> 2. location of Flo-Klor(outdoor chemical storage), ammonia diffusion water for pump down (outside, near diffusion <br /> tank), and glycol (4 drums observed near south fence) <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: Immediately log into the California Environmental Reporting System (CERS)at <br /> http://cers.calepa.ca.gov/, upload the correct or updated information, and submit to the EHD for approval. <br /> This is a repeat violation, Class II. <br /> 9 HSC 25508.2 Failed to annually review and certify that the HMBP is complete and accurate by the due date. <br /> OBSERVATION: The business plan information was not reviewed and resubmitted in the California Environmental <br /> Reporting System (CERS)annually by the due date,January 15. The business plan was submitted late in 2021 and <br /> 2022. <br /> REGULATION GUIDANCE: The hazardous materials inventory shall be submitted by January 15 of each calendar <br /> year and may be submitted beginning November 1 of the previous year <br /> CORRECTIVE ACTION: Submit a statement to SJC EHD stating that the business will annually certify/submit the <br /> annual HMBP to CERS by January 15. <br /> This is a repeat violation, Class II. <br /> Overall Inspection Comments: <br /> An inspection checklist was provided to the facility operator on the day of inspection.
