The following is an itemized list of underground storage tank violations that have not been addressed for GRANT LINE SHELL as of April 29, 2024. Open violations from May 30, 2023 inspection Violation#209-Failure to maintain a copy of the"Facility Employee Training Certificate"on-site. OBSERVATION:The Facility Employee Training Certificate dated 12/23/2022 list incorrect initial training dates and the"Assuming Responsibility as an Employee"are not listed. REGULATION GUIDANCE:All site employees must be trained prior to taking responsibility as a facility employee if after October 1,2018, or within 30 days of hire if prior to October 1,2018. The Facility Employee Training Certificate shall contain accurate information for all employees trained. The template provided as a sample by the inspector was shared with all local DO inspectors for future reference. OBSERVATION:The designated operator employee training for Shannon Vanluchem was not performed. REGULATION GUIDANCE: The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include,but is not limited to: 1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3.Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training.
