The following is an itemized list of CP22 - UST FULL violations that have not been addressed for Nashir Hunter Inc. as of July 11, 2024. Open violations from May 29, 2024 inspection Violation#57-2030012-Initial and annual training provided to employees and at least one trained employee is present. OBSERVATION: The designated operator employee training for Aaron Lopez was not performed. REGULATION GUIDANCE:The designated operator shall train facility employees in the proper operation and maintenance of the UST system at least once every 12 months. The training shall be conducted before the individual performs the duties of a facility employees.The training shall include, but is not limited to:1. Operation of the UST system in a manner consistent with the facility's best management practices 2. Employee's role with regard to monitoring equipment as specified in the facility's monitoring plan 3. Employee's role with regard to spills and overfills as specified in the facility's response plan 4. Name of the contact person(s)for emergencies and monitoring equipment alarms Initial training shall also include a site-specific practical demonstration that provides both sharing information and evaluating knowledge for responding to emergencies, leak detection, monitoring alarms, spills and overfills. At least one of the facility employees present during operating hours shall have current training. Monitoring records include: (1)date and time of all monitoring or sampling; (2)monitoring equipment calibration and maintenance records; (3)results of any visual observations; (4)results of sample analysis performed a lab or in the field; (5)logs of all readings of gauges or other monitoring equipment,ground water elevations, or other test results; (6)results of inventory readings and reconciliations. CORRECTIVE ACTION: Immediately locate and maintain all missing maintenance and monitoring records for the last three years on site.Submit copies of any missing work orders and alarm records to the EHD.
