<br />4 of 4 <br />Environmental Health Department <br /> <br />PRIOR TO FIRST FUEL DELIVERY: <br /> <br />8. A valid California Department of Tax and Fee Administration (CDTFA) number must be obtained and <br />provided to EHD prior to fuel drop. This number must be uploaded into the California Environmental <br />Reporting System (CERS) at https://cers.calepa.ca.gov. <br /> <br />9. Submit to CERS a complete UST submittal. The UST submittal must be approved by EHD prior to <br />receiving approval for fuel delivery. <br /> <br /> <br />WITHIN 30 DAYS OF RECEIVING FUEL IN THE UST SYSTEM: <br /> <br />10. Submit to CERS all applicable hazardous materials (HMBP) elements within 30 days of handling an <br />undisclosed hazardous material. The HMBP submittal must be approved by EHD. <br /> <br />WITHIN 30 DAYS OF INSTALLING THE UST SYSTEM: <br />11. The “UST Certification of Installation/Modification” element in CERS shall be submitted for installed tank <br />systems, pursuant to CCR 2635(f). <br /> <br />PRIOR TO APPROVAL TO OPERATE: <br />12. The following items must be completed prior to receiving approval to operate: <br /> <br />a. An accurate and to-scale as-built diagram or drawing, which indicates the location of the UST <br />system (underground storage tanks, piping, auxiliary equipment) with respect to buildings or other <br />landmarks, per CCR 2711(a)(8), must be submitted to EHD. <br /> <br />b. Completed copies of all required installation checklists (tank, etc.) must be submitted to EHD. <br /> <br />c. The designated operator (DO) training requirement must be satisfied, per CCR 2715(c). Submit <br />a copy of the completed Facility Employee Training Certificate. <br /> <br />d. All outstanding operating permit and service request fees must be paid prior to receiving the final <br />approval to operate. <br /> <br /> <br />This permit expires at the end of the calendar year.
