The unsigned dates include:
 May 2, 13, 27, and 30, 2025
 April 11 and 25, 2025
 August 1, 2025
Additionally, it was noted that several manifests, my name—Dawn Kent—was placed across the page
and referenced with the phrase “per Dawn Kent.” This is not acceptable, as the manifests should be
signed by an authorized EVS staff member, not attributed to me unless I am directly involved. One
manifest also included a note stating that the gate was closed which needs clarification.
I look forward to hearing back from you with a solution to ensure proper documentation and compliance
moving forward.

Thank you,
Dawn Kent
Manager, Environmental Services
Sutter Tracy Community Hospital
dawn.kent@sutterhealth.org
209-832-6053 (Office) 209-371-7078 (Mobile)
