HSC 117960 - LQG shall file MWMP with the EA <br /> DBSERVATION: Medical Waste Management Plan was received by the EA during inspection, however the MWMP is lacking <br /> nformation. <br /> :,ORRECTIVE ACTION: Provide the following information in the MWMP for this facility: Phone Number, Business Type, <br /> mergency Action Plan, Closure Plan, and Operator Certification. <br /> 13. HSC 117975- LQG shall maintain treatment and tracking documents for 2 years.A large quantity medical waste <br /> generator required to register pursuant to this chapter shall maintain for a minimum of two years individual treatment <br /> records and the tracking document for all untreated medical waste shipped offsite for treatment.T <br /> OBSERVATION: The tracking and treatment record from 3-7-22 was not available for review since the hauler Stericycle does <br /> not have a record for that date. <br /> CORRECTIVE ACTION: Provide the record or explain how the site will be sure to store biohazardous waste for no more than <br /> seven days per HSC 118280(e)(1)(A). <br /> 25. HSC 118280 - Biohazard bags shall be containerized and stored pursuant to this section. (A) If the person generates <br /> 20 or more pounds of biohazardous waste per month, the person shall not contain or store that waste above 0' <br /> Centigrade (320 Fahrenheit) at an onsite location for more than seven days without obtaining prior written approval of <br /> the enforcement agency <br /> OBSERVATION: Tracking and treatment records were observed for 12-6-2021 and then the next pick up was 12-23-2021. <br /> CORRECTIVE ACTION: Provide tracking and treatment documentation of pick up on 12-13-2321. <br /> NOTES: <br /> 1. Report emailed to sandeepkaur(oD-redcross.org <br /> 2. Waste hauler is Stericycle. <br /> 3. Facility may qualify as a small quantity generator. Medical Waste Management Plan and application given to Quality <br /> Assurance Associate <br /> 4. Biohazard bags meet ASTM D 1709 and ASTM D 1922 requirements.. <br /> 5. A Medical Waste Application Packet and form 5021 was given to the Quality Assurance Associate during inspection. <br /> CORRECTIONS DUE BY 12/30/2023 <br /> Reinspection on/about: A reinspection fee of$162 per hour may be charged.
