Hazelton Ave <br /> Stockton, CA 95205 <br /> Dear Ms. Ramirez, <br /> This letter is in response to the Medical Waste Inspection Report observation issued Tuesday, October 14, <br /> 2025, in reference to the American Red Cross (ARC) facility located at 65 N. Commerce, Stockton, CA <br /> 95202. <br /> Observation: <br /> There is a discrepancy between the generator and hauler pickup date for the 4/17/25 tracking and <br /> treatment record. <br /> Upon receipt of the Medical Waste Inspection Report, employee CC was questioned to determine if there <br /> were any factors that contributed to the incorrect date entry recorded. Employee CC has been primarily <br /> responsible for the reconciliation of the waste management shipping manifest since employment in 2017. <br /> Of the 115 waste management shipping manifests reviewed during this audit,this one documentation error <br /> (4/7/25 recorded instead of 4/17/25)was the only issue identified. The investigation revealed that the <br /> error was an isolated oversight. <br /> On October 21,2025,the staff involved completed the annual Regulated Medical Waste training(See <br /> Attachment I). <br /> Please be assured that the American Red Cross takes these matters seriously and has thoroughly <br /> investigated the factors that contributed to the documentation error.
