Date: 12/3/2026 <br />Manufacturer training: VR, 12/02/26; OPW, 11/11/26 <br />OVERALL INSPECTION NOTES AND COMMENTS <br />NOTES <br />Attempted a pre to first fuel delivery inspection. The facility was not prepared to proceed due to the following: <br />- Required testing equipment was missing, including hoses and valves necessary to verify communication at each endpoint of the <br />vacuum zone. <br />- Monitoring system paper was not available. <br />- The technician was unfamiliar with the procedure for testing communication. <br />- Equipment required to remove overfill prevention devices was not available. <br />- The technician had difficulty retrieving manufacturer certifications. <br />- The technician was not informed of the various inspection types. He stated that he had only received the UST Installation <br />Progress Sheet prior to my arrival and was unaware of the inspection checklist. He was advised that a pre -monitoring certification <br />inspection was required. <br />This facility is not approved to receive fuel. <br />This facility is not approved to operate. <br />Additional inspections are required. Refer to the approval letter dated June 14, 2024, for a full list of outstanding requirements. <br />Please submit inspection requests in writing to UST@sjgov.org with 48 hours advance notice. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
