A dOAQU1N Environmental Health Department <br /> COUNTY <br /> CP22 - AST Non-Qualified Inspection Report <br /> Facility Name: Facility Address: Date: <br /> A&A CONCRETE SUPPLY INC 4035 E MARIPOSA RD, STOCKTON June 27 2024 <br /> SUMMARY OF VIOLATIONS <br /> (CLASS I,CLASS II,or MINOR-Notice to Comply) <br /> Item# Remarks <br /> CORRECTIVE ACTION: Ensure the SPCC Plan is updated so that portions describing procedures to be used when a <br /> discharge occurs for each type of major equipment failure, specifically 350-gallon waste oil tank,will be readily usable <br /> in an emergency. Submit proof of correction to the EHD. <br /> This is (Class 2)Violation. <br /> 30 Code of Fed Regulations 112.7(f)(1)4020001 -Training provided for op/maint of equip, discharge procedures, <br /> laws/regs, general fac ops, and SPCC. <br /> OBSERVATION: It appears that the oil handling personnel were not adequately trained, as required. Facility failed to <br /> provide training to the oil-handling personnel regarding the following: 1.The operation and maintenance of equipment to <br /> prevent discharges. 2. General facility operations. 3. The contents of the SPCC Plan. <br /> -Appendix B monthly training form was not being used as stated in the SPCC Plan. <br /> -The 240-gallon hydraulic and motor oil tanks are to have all sides visible per the SPCC plan to conduct monthly <br /> inspections(Section 15.0 and 3.0 in SPCC Plan)but was observed on site located against a wall with items around, <br /> not allowing full access to sides and no visual access to the back of the tanks. <br /> REGULATION GUIDANCE: (f)(1)At a minimum,train your oil-handling personnel in the operation and maintenance of <br /> equipment to prevent discharges; discharge procedure protocols; applicable pollution control laws, rules, and <br /> regulations;general facility operations; and,the contents of the facility SPCC Plan. <br /> CORRECTIVE ACTION: Immediately provide adequate training to all oil handling personnel and submit a copy of the <br /> training log and training content to the EHD. <br /> This is (Class 2)Violation. <br /> 32 Code of Fed Regulations 112.7(f)(3)4020002-Spill prevention briefings are conducted annually. <br /> OBSERVATION: Failure to conduct spill prevention briefing for oil-handling personnel at least once a year to assure <br /> adequate understanding of the SPCC Plan. <br /> -Training records related to the SPCC Plan were not observed on site. <br /> REGULATION GUIDANCE: (f)(3)Schedule and conduct discharge prevention briefings for your oil-handling personnel <br /> at least once a year to assure adequate understanding of the SPCC Plan for that facility. Such briefings must highlight <br /> and describe known discharges as described in§ 112.1(b)or failures, malfunctioning components, and any recently <br /> developed precautionary measures. <br /> CORRECTIVE ACTION: Discharge prevention briefings for oil handling personnel must be scheduled and conducted at <br /> least once a year to assure adequate understanding of the SPCC Plan for that facility. Conduct all required spill <br /> prevention briefings. Provide proof correction to the EHD. <br /> This is(Minor)Violation. <br /> 57 Code of Fed Regulations 112.7(e), 112.8(c)(6)4010021 -Written Inspection/testing procedures kept, inspection/testing <br /> records signed, kept for 3 years. <br /> OBSERVATION:The facility failed to comply with the following requirements: <br /> 1. Have a record of inspections and integrity tests signed by the appropriate supervisor or inspector. 2. Keep written <br /> procedures and records of inspections and integrity tests for at least three years.
