This Form is required to be filled out and emailed back to me. <br />The Authorized Representative Form <br />1. This form authorizes you as the "Authorized Representative" for the permit. Please <br />include a separate form for each Authorized Representative if there is more than one. <br />Authorized Use of Property Form <br />1. This form is required to be completed if you are not the property owner and are leasing <br />the property. <br />There are a few other required items that must be included with your Op -ID: <br />A site map indicating the location of the operation <br />1. If you give me a google earth map or any map of operation, I can make a more <br />detailed map for you using our GIS software. <br />A copy of your Certified Applicator Certification/License (QAC/QAL) issued from The <br />Department of Pesticide Resources (DPR) or a copy of your Private Applicator Certification <br />(PAC aka "Brown Card") issued from a County Agricultural Commissioner. <br />Again, I will send you a couple of different emails with additional requirements to help you keep track of <br />the documents.
