San Joaquin County Environmental Health Department Program <br /> 1868 E.Hazelton Ave.,Stockton,CA 95205 Record: PR0518328 <br /> 1209)468 3420 Program <br /> • 'g4(coR`��P www.s000v.org/ehd Element: 4520 <br /> Observations and Corrective Actions: <br /> 13.HSC 117975 -LQG shall maintain treatment and tracking documents for 2 years. <br /> OBSERVATIONS: <br /> Treatment and tracking records were missing for the following dates: <br /> • September 27, 2023 to October 11, 2023 <br /> • October 25, 2023 to November 22, 2023 <br /> • November 29, 2023 to December 20, 2023 <br /> CORRECTIVE ACTIONS: <br /> A large medical waste generator required to register with the EHD shall maintain individual treatment operation records, and <br /> shipping and tracking documents for all untreated medical waste shipped offsite for treatment for 2 years. Provide treatment and <br /> tracking records for the dates mentioned above to the EHD. If waste was not picked up during those dates, then ensure it is <br /> properly documented. <br /> 25. HSC 118280 -Biohazard bags shall be containerized and stored pursuant to this section. <br /> OBSERVATIONS: <br /> 1.A biohazard container located in the Lab-Outpatient Exam Room was not properly labeled (see PHOTO 1). <br /> 2. Biohazard waste was kept on site over 7 days between the dates of February 16, 2022 to February 24, 2022 and August 30, <br /> 2023 to September 7, 2023. <br /> CORRECTIVE ACTIONS: <br /> 1. Biohazardous waste shall be bagged and placed in a rigid container which is leak resistant, have tight-fitting cover, and be <br /> kept clean and in good repair. Container shall be labeled with the words"Biohazardous Waste" or"BIOHAZARD" on the lid and <br /> on the sides so as to be visible from any lateral direction. Properly label the container and provide photographic evidence of <br /> correction to the EHD. <br /> 2. A person who generates 20 or more pounds of biohazardous waste per month shall not contain or store that waste above 32 F <br /> onsite for more than 7 days.A person who generates less than 20 pounds of biohazardous waste per month shall not contain or <br /> store that waste above 32 F onsite for more than 30 days. Ensure biohazardous waste is removed from the facility within 7 days. <br /> Correct all violations and provide evidence of corrections to the EHD <br /> by February 29, 2024. <br /> Notes: <br /> 1. Inspection report emailed to Chandra N. Buchanan, Sr. EH&S Manager/Safety Leader, at Chandra.n.buchanan(o)-kp.org. <br /> 2. Biohazard bags meet ASTM—D1709 and ASTM—D1922 requirements. <br /> 3.
