The site map has not been updated to match the new business layout. <br /> The site map is missing the following elements: <br /> -location of propane, acetylene, ethyne <br /> -location of fire extinguishers <br /> REGULATION GUIDANCE: site map that contains north orientation, adjacent streets, access and exit points, <br /> evacuation staging areas, hazardous material handling and storage areas, emergency response equipment, and, if <br /> present, loading areas, internal roads, storm and sewer drains, and emergency shutoffs, as well as additional map <br /> requirements the governing body of the unified program agency finds necessary. <br /> CORRECTIVE ACTION: This was corrected on site but uploading updated maps into CERS. <br /> This is (Minor)Violation. <br /> 12 CA Health and Safety Code 25505(a)(3) 1010010-Electronically submitted adequate response plan/procedures for <br /> release/threatened release of hazmat. <br /> OBSERVATION:The emergency response(ER)procedures were lacking or inadequate.The following were missing: <br /> -procedures for release in Section B <br /> -CUPA phone number <br /> -locations of emergency equipment <br /> REGULATION GUIDANCE:A business plan shall include the following emergency response procedures for a release <br /> or threatened release of hazardous materials, including, but not limited to, the following: -immediate notification of <br /> local emergency personnel and the Environmental Health Department(EHD) -procedures for the mitigation of a <br /> release or threatened release to minimize any potential harm or damage to persons, property, or the environment - <br /> evacuation plans and procedures, including immediate notice,for the site <br /> CORRECTIVE ACTION: This was corrected on site but uploading an updated contingency plan into CERS. <br /> This is(Class 2)Violation. <br /> Overall Inspection Comments: <br /> Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br /> documenting the corrective actions that have been or will be taken for each violation, and any supporting <br /> paperwork, by 7/17/2024 . <br /> To minimize person to person contact EHD is choosing to write the name of person receiving the report instead of <br /> having them sign.
